07 - Confusing the hell out of Angie



(Music: Routine)





(It almost felt like time was slowing down... Did I really do that? I knew exactly what to do, but I don't know how I knew that... Ugh, now I'm just confusing myself--)

"Derek!"

"Pay attention!"

"Huh? I-I mean, I was!"

"Stop daydreaming and start taking your job seriously!"

"I'm sorry."

"We need to have a meeting about Mr. Pratt's operation. Angie, can you prep for that?"

"Yes, of course, Doctor."

(Music: Patient Profile)



"He's been suffering from dizziness and nausea recently. Tests show that wastes accumulated in his spleen, and are now blocking blood vessels there. Today's objective is to remove the buildup of thrombi. The contrast medium was administered thirty minutes ago in IV. Because of that, you should be able to find the waste using ultrasound. Once it shows a shadow, try clipping near that area with the forceps. It should stop moving. Make your incision, then drain the area. This is a sensitive operation, so you'll need the magnifier."

"So, remember, the objective is: -Remove all thrombi in the vessels. That is all. If further waste blockage occurs, the patient's vitals will drop."

"Remember to work quickly, but with precision!"

The patient's name this time is taken from Dr. Pratt from ER, as well as Dr. Chase's actor from House, Jesse Spencer.

"These readings aren't good... treat the spleen right away."



"The thrombi causing the vessel blockage are originating here. Work carefully: use the magnifying tool to view the vessel area."





What you must do here is find the thrombi with the ultrasound, grip it at the very front with the forceps (if you try in the middle, it will slip through and continue its path through the blood stream), slice it with the scalpel, and drain it. Doing two at a time is ideal because if one slips through, vitals drop by ten points or so.





I get them all, but then...



More thrombi begin to rush through the organ at three times the speed.

"The change in blood flow seems to be causing the suspended thrombi to move, as well! Doctor, this is bad!"

(Music: Emergency!)

This operation is a little unfair because depending on where you unfreeze the dialogue, vitals may plummet by 40 or more, or not at all. If they do, kiss your S Rank goodbye.





Instead of having to work at a very fast speed, now you must remove the thrombi in slow motion. Easy. There are seven to do.



Angie was about to say "What are we going to do?" And then she realized the operation was over.

"N-No thrombi detected... treatment is complete... Suture the incision and the operation will be complete."



In the video, I S rank the mission. The requirements are to not let four or more thrombi pass through the spleen - If you read what Angie has to say after the thrombi break through, you'll lose the S Rank. You also must complete the operation in less than 1 minute and 20 seconds.

"Huh?"

"You were practically a different person in there.. Just like yesterday."

(I did feel... almost like I was someone else.)

"If you were that focused more often, I wouldn't have to worry as much."

"So you worry about me?"

"I definitely worry about your patients!"

*sigh*

"Angie, I'd like to review the footage of today's operation."

"Of course, sir. I'll bring it to you later."
